Appointments

A scheduled appointment is required to be seen in our office. Appointments can be scheduled with any of our providers by calling (310) 337-7171. Appointments with our Physician Assistants are generally available in about 1 week and appointments with Dr. Sofen are generally available in about 3 weeks. Availability varies throughout the year.

A wait list is available for patients who wish to be seen sooner than their scheduled appointment. Please provide your daytime phone number when asking to be placed on our wait list. We are able to offer appointments to patients on our wait list on a first-come, first-serve basis as cancellations are made.

Patients arriving more than 15 minutes late may need to reschedule their appointment. Please call our office to notify the front desk if you expect to arrive late to your appointment. If you are unable to make an appointment at your scheduled time, please provide at least 24 hours’ notice when cancelling. If you do not give sufficient notice, a fee of $25 may be charged.

What to bring

Insurance Card: All new patients must bring their insurance cards. Any time your insurance information changes, be sure to bring the new card so we can update your information.
Authorization / Referral: If you have an HMO, each and every visit requires authorization for you to be seen. Please be sure to bring such authorizations with you.
Co-pay: This amount is generally found on your insurance card. If multiple co-pay amounts are listed, contact your insurance to ask which is for specialists. We accept cash, checks, and credit cards (Visa, Mastercard & American Express).
New Patient Forms: We have posted our forms online for those patients who wish to print the forms themselves and bring them at the time of their appointment. Forms will be provided at the front desk if you would rather complete them in the office.
A guardian: If you are not of legal age, you must have a guardian present for your first appointment. After your initial appointment, your guardian can complete a Consent to Treat an Unaccompanied Minor form.

Insurance Coverage

Our office participates in most PPO health plans and is contracted with about two-dozen Medical Groups. We accept patients with Medicare and straight Medi-Cal. If your insurance company requires you to be referred to a specialist by your primary care physician, a valid referral must be obtained prior to your visit.

Though we will submit your claim to the insurance company we have on file, we are ultimately not responsible for lapsing coverage, changing coverage, termination, or lack of payment due to not-covered procedures. Please remember, verification of insurance coverage does not guarantee that the patient will not be accountable for some portion of the charges.

Payment Policy

All copayments and deductibles are due at the time of service. All previous balances must be cleared at the time of services. Patients are responsible for paying all balances not paid by the insurance carrier. If insurance coverage has been terminated prior to the time of service, patients are responsible for all charges incurred.

We accept cash, checks, and credit cards (MasterCard, Visa, and American Express) for all payments. ATMs are accessible at banks next door and across Sepulveda Blvd.
